Rancangan Formulir Rekam Medis Bencana

Abstract
Kejadian bencana alam setiap tahunya terus meningkat yaitu banjir, puting beliung, dan tanah longsor, mengakibatkan lebih dari 8,6 juta jiwa menderita dan mengungsi, serta 676 jiwa meninggal, sementara 370 fasilitas hancur, sehingga penting untuk merancang formulir rekam medis bencana yang sederhana, ringkas, dan akurat untuk meningkatkan kesiapsiagaan bencana. Penelitian ini bertujuan untuk merancang formulir rekam medis bencana yang sederhana dan efektif guna meningkatkan pengumpulan data dalam situasi darurat. Metode yang digunakan adalah Research and Development (R&D) dengan melibatkan ahli untuk merancang formulir rekam medis bencana. Uji formulir rekam medis bencana dilakukan dengan korelasi Pearson product moment oleh 25 petugas rekam medis di rumah sakit, dengan taraf signifikansi 0,05% dan nilai r tabel 0,413. Hasil penelitian menunjukkan bahwa formulir yang dirancang memenuhi kriteria validitas dan reliabilitas, dengan nilai r hitung yang signifikan di atas r tabel dan nilai Cronbach-Alpha sebesar 0,947. Formulir ini diharapkan dapat meningkatkan efisiensi pengumpulan data medis saat bencana, serta memastikan kesinambungan perawatan pasien dalam kondisi darurat.
